Click the yellow + in top right hand corner, select ‘Log incident’.
There are 5 stages, as described below. Fill in all the information, as comprehensibly, as possible in stages 1-5.
Stage 1 asks about the location of the incident, the date and time, the person logging the incident, the type of incident and any involved assets.
The user should select the most appropriate incident category, if ‘some other type of incident’ is selected a second drop down with further categories becomes available. If a food related incident is selected, further fields are made available in stage one to capture information such as symptoms and how the premises was made aware of the allegation.
Stage 2 allows details of people involved in the incident and witnesses to be added. If a person involved in the incident is a registered Saeker user, then the dark blue ‘Add User’ button will allow the user to be selected from a list. If the person involved in the incident is not a Saeker user, a member of the public for example, the green ’Add person’ box should be selected. Any witnesses, who were not directly involved should be added as a witness, along with any statements they gave.
Stage 3 allows full details of what happened to be entered and allows relevant documents to be uploaded. Documents can be videos, photos, PDFs or any other style of computer file.
Stage 4 allows external authorities to be added to the incident report, such as if the police or fire brigade became involved.
Stage 4 also allows actions to be raised and allocated to other Saeker users, to prevent re-occurrence or to minimise further impact from the incident.
Stage 5 allows users to be selected to receive a notification about the incident. It may be appropriate to notify the area manager or the insurance company for example. These notifications are in addition to the employees who have been selected to receive all notifications for that premises.
This is the final stage of the report, once all information has been entered the employee should select save and the report will be completed. The employee will be re-directed back to their dashboard.
